2020年扶老比已達38.96%、老化指數達119.82%,隨人口結構轉變使得機構長者照護品質受重視,照護者壓力與日俱增。長照機構內護理人員除滿足長者的身心靈照護、與照護專業團隊溝通協調、串聯長照管理單位及社區長照資源連結,甚至與長者家庭斡旋。當工作量增加了,職場疲勞及態度對照護老人帶來負面結果,致護病關係僵化,離職意願提升。本研究擬探討長照機構護理人員職場疲勞與對老人態度及照顧意願間之關聯,藉以協助政府及相關單位提升老年人受到的照護品質,也友善護理人員的工作環境。
本研究以橫斷式問卷調查法(Cross-sectional),針對長照機構護理人員年齡滿20歲、可了解問卷內容且配合完成資料收集者之護理人員為研究對象,並排除有精神疾病史者、拒絕參與者。本研究結果發現,長照機構護理人員職場疲勞之程度以輕度居多,工作相關過勞大於個人相關過勞;年齡、修過老人護理學、照護老人比例與與職場疲勞顯著相關;老人同住與否、畢業時間至今時間與照顧意願顯著相關;是否修過老人護理學、照顧比例、畢業時間與個人過勞顯著相關;長照機構護理人員對老人態度是影響照顧意願之重要預測因子,尤其生理及人際面向。長照機構護理人員過去多數有照護老人經驗(94.6%)並與老人同住經驗(84.6%)。有與老人同住經驗者,其對老人態度表現越高,照顧意願度高。老人照顧方面的問題,需要照顧者更多瞭解與認知,而具有與老人同住經驗者或是曾經修習過老人照顧學等,正是彌補了照顧者對老人的照顧知識或心理不足之處。建議護理人員在職進修加入老人護理相關課程;機構開辦紓壓課程,以減輕其職場疲勞,進而使留任率提升;政府鼓勵舉辦「在學青年換宿」等活動使各年齡層接觸老人機會增加,投入照護工作,從而減少長照人力缺口。

In 2020, the old age dependency ratio has reached 38.96%, and the aging index has reached 119.82%. As the population structure changes, the quality of care for the elderly in long-term care institutions has been valued, and the pressure on caregivers is increasing. Nursing staff in long-term care institutions not only meet the needs of the elders’ physical, mental, and spiritual care, communicate and coordinate with professional care teams, connect with long-term care management units and community long-term care resources, and even mediate with the elderly’s families. When the workload increases, workplace fatigue and attitudes bring negative results to caring the elderly, resulting in a rigid relationship between nurses and patients, and an increase in willingness to leave. This study aims at exploring the relationship between workplace fatigue of nursing staff in long-term care institutions and their attitude and willingness to care for the elderly, so as to assist the government and related units to improve the quality of care received by the elderly, and also to improve the working environment of nursing staff. This study uses a cross-section survey, targeting nursing staff in long-term care institutions who are over 20 years old, could understand the contents of the questionnaire and cooperate to complete the data collection as the research object, and those who with a history of mental illness and who refused to participate were excluded. The results of this study revealed that workplace fatigue among nursing staff in long-term care facilities was mostly mild, and work-related fatigue was greater than personal-related fatigue; age, taken gerontological nursing, and ratio of caring elderly are significantly related to workplace fatigue; whether lived with elderly people or not, and time since graduation are significantly related to willingness to care; whether gerontological nursing were taken, ratio of caring elderly, and time since graduation are significantly associated with personal-related fatigue; and attitude toward elderly people among nursing staff in long-term care institutions is an important predictor of willingness to care, especially physiological and interpersonal aspects. The majority of nursing staff in long-term care institutions had experience in caring for the elderly (94.6%) and living with the elderly (84.6%). Those who have lived with the elderly showed higher attitude towards the elderly and higher willingness to take care of them. The care of the elderly requires more knowledge and awareness of caregivers, and those who have experience living with the elderly or have studied gerontological nursing, etc., make up for the lack of knowledge or psychology of care for the elderly. It is suggested that nursing staff could take courses related to elderly care in their in-service training; institutions should offer courses about stress reliefing to reduce workplace fatigue of nursing staff and enhence their intention to stay; the government encourages institutions to organize activities such as "student work-for-living" so that all age groups can have more opportunities to get in touch with the elderly and engage in care work, thus reducing the shortage for long-term care.
